The insurer or leasing company can provide you with a new paper accident report form free of charge, often sent by post. Some leasing companies also offer a downloadable version. If your employer works with a fixed insurer, you can request a paper copy directly from them — usually via their website or customer service.
In Belgium, the official app “Mobiel Schadeformulier” and in the Netherlands “Mobielschademelden.nl” can be used as digital alternatives. These are officially recognized and can replace the paper form, provided all involved parties agree.
